Bernalillo Head Start provides high-quality early education for 76 children ages three to five years of age in partnership with their families and the community. The vision of Bernalillo Head Start is that every child enrolled in Head Start will enter kindergarten with the competencies necessary to succeed in school and parent/caregivers will be empowered to be their child's most important teacher. Home Visiting Program: Home-Based services are provided to 38 families in Sandoval County. The Home Visiting Program is a year-round program offered to families with children ages birth to three years. It provides an opportunity for parents to encourage their child's healthy development. Together, the parents and Home Visitor create a home learning environment that will help the child grow and develop socially, intellectually, emotionally and physically. |

Behavioral Health: Carlsbad Family Health Center works in conjunction with Carlsbad Mental Health Center to provide high quality behavioral health services to those suffering with emotional issues as well as severe disabling mental illness. These services include assessments, individual counseling, family counseling, marriage counseling, crisis intervention, medication services by referral to on-site physicians. Also available by referral to Carlsbad Mental Health Center are psychosocial rehabilitation services, substance abuse programs, anger management groups, comprehensive community support services, psychiatric consultation services, and more intensive medication management services. |

Supportive Living:
The Northwest New Mexico Hospice Team provides skilled care for the terminally ill and their loved ones. Patients and their families are included in the decision making process concerning the plan of care for the patients' physical needs along with their emotional and spiritual needs. Bereavement counseling is available to the family before and after the death of their loved one. The Northwest New Mexico Hospice Team believes quality of life to be as important as the length of life. As such, their goal is to help a person's last months to be as fulfilling and comfortable as possible through palliative care. The Northwest New Mexico Hospice team is comprised of dedicated professionals including a licensed medical director, registered nurses, home health aides, physical therapist, medical social worker, bereavement counselor, chaplain and volunteers. Hospice offers help and support to the patient and family on a 24-hour-a-day, seven-days-a-week on-call basis. Northwest New Mexico Hospice is proud to provide the only not for profit in-home hospice service to the people of San Juan County since 1987. |

Since 1982, Project Shield has been committed to responding to the developmentally disabled adult employment and community inclusion needs of San Juan County residents. The service area includes the communities of Farmington, Aztec, Bloomfield, the San Juan portion of the Navajo Reservation and all other areas of the County. Project Shield works with clients, guardians, and the community to provide a range of individualized employment and community inclusion related services to persons with developmental disabilities. Each client has an Individualized Service Plan that helps staff address the unique needs of each client and provide the most effective support possible. Services are provided through the following programs: New Mexico Developmental Disabilities Waiver. Day Habilitation: Habilitation services help maximize independent living skills in the areas of self care, physical and emotional maturity, and social communication. Staff is trained to assist clients in areas of money management, community integration, individual relationships, educational pursuits, personal care, and other areas identified in the clients Individualized Service Plan. Community-Supported Employment: This employment model provides job development, placement, on-the-job training, and on-going support to individuals in community employment. All clients receive service coordination and transportation services, as well as assistance from a Job Coach who consistently evaluates job placement and support satisfaction. Project Shield partners with over 20 employers in San Juan County, allowing the program to offer clients job placements with a wide variety of skill level. Community Access: Project Shield has an intensive program to identify and implement natural supports for all clients. Natural supports consist of non-paid volunteers or clients who provide friendship and social interaction to other clients. Often volunteers take clients to a movie or other social activity and assist in linking clients to resources in the community that promote independence. Non-Medical Transportation: Transportation services are provided to all clients to and from the Project Shield facility, employment settings, social activities, shopping, etc. |

Other services available by referral are: Behavioral Health, Substance Abuse Programs, Mammography and Specialty Care. Anyone is eligible to receive services at Hobbs Family Health Center regardless of ability to pay. Sliding scale fees are set based upon the total household income and size of family, according to proof of income. In addition to private pay, we accept Medicaid, Medicare or private insurance. Our office will file the insurance forms for our patients. Otherwise, payment is expected at time of service. Children's Services: Home Visitor Program offers free services to all first time parents in Lea County. These services may begin during the prenatal stages and follow the family up until the child is 3 years of age. We believe parents are their child's first and most influential teacher therefore our mission is to provide the information, support and encouragement parents need to help their children develop during the crucial early years of life. A certified parent educator will provide weekly home visits at the family's convenience. During these visits the parent educator will provide information about the child's developmental milestones, perform development and health screenings and provide referrals to community resources. |

Dedicated providers offer an unprecedented level of expert, comprehensive and personalized care for individuals at the end of life. The teams include Medical Directors, Nurses, Medical Social Workers, Home Health Aides, Physical Therapists, Pastoral and Grief Support Counselors, and volunteers. Each member of the team is part of a circle of care that surrounds every individual served by The Hospice Center. |
