Primary Care:
Comprehensive family medical care includes: Chronic Illness Care, such as diabetes, high blood pressure and asthma, acute illness care, preventive medicine, family planning, women's health, wellness physicals and screenings. We also run a Hepatitis C program in collaboration with UNM - Project ECHO. Behavioral Health: Artesia Health Resources works in conjunction with Milestones Wellness Center to provide high quality behavioral health services to those suffering with emotional issues as well as severe disabling mental illness. These services include assessments, counseling, psychosocial rehabilitation, psychiatric consultation via telehealth, substance abuse programs, anger management groups, marriage counseling, family counseling, comprehensive community support services, and crisis intervention. Dental Services: Our dental program offers comprehensive dental services. We accept Medicaid for adults. Our dental services consist of oral health screenings, cleanings, basic and major restorative care, and denture services. |

Primary Care:
PMS/Checkerboard Area Health System (CAHS) is a system of 5 clinics located in and around Cuba, N.M. We provide an integrated program of primary care, dental care, and behavioral healthcare. We provide a large range of services, including, but not limited to diabetes education and management, hypertension management, chronic disease management, acute care issues, prenatal, pediatric, and immunizations. We have special contractual agreements which allow us to provide optometry care and high risk OB care. We also own and operate the only certificated ambulance service in Northern Sandoval County, which services the community within a 50 mile circumference around the Cuba Health Center. CAHS has a capitated contract with Indian Health Services to provide healthcare to all Native Americans who reside within our contract care service area. Through this contract we provide Public Health Nursing and Utilization Review for our IHS patients. We receive funding through a multitude of sources, including most private insurances, Medicaid, Medicare, Federal 330 grants funding, RPHCA funding, Indian Health Care funding, as well as, Sandoval County and State funding for the ambulance program. Behavioral Health: Our Behavioral Health care provides individual, group, couples, child therapy, substance abuse assessment and psychiatric diagnosis and medication management via tele-psych. Dental: Our dental clinics provide care for both adults and children. Our dental services consist of oral health screenings, cleanings, basic and major restorative care, and denture services. |

Primary Care:
FCHC provides primary medical services to the residents of San Juan County. All our Medical Providers are board certified in Family Medicine or Pediatrics. We offer a full range of primary care services such as; preventive care (well child, immunization, health education, and well woman), acute care, chronic disease management (diabetes, hypertension, asthma, COPD and Hepatitis C), women's health and occupational health. We accept all insurances and offer a sliding fee schedule for uninsured and underinsured patients who meet income guidelines. We also provide lab and pharmacy services on site. Behavioral Health: FCHC provides a wide range of Behavioral Health and Substance Abuse treatment services for adults and children. Services include; assessment, counseling, community support services, psychosocial rehabilitation, assertive community treatment, crisis intervention, medication management, psychiatry, drug court, and community corrections. We accept all insurances. Dental Services: FCHC provides dental services to residents of San Juan County. Our Dentists and Dental Hygienist are board certified. Services we provide include; preventive services, emergency services and restorative services. We provide services for both adults and children. Our staff has special expertise in providing dental services to the disabled and the elderly. We accept all dental insurances and provide a sliding fee scale for patients who meet income guidelines. Other Services: Transportation/Facilities Dept - San Juan Red Apple Transit operates Monday - Saturday, 7:00AM - 6:00PM Holidays that Red Apple Transit does not operate: New Year's Day, Martin Luther King Day, Presidents Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day. Dial-A-Ride demand response service available on a first-come, first-served basis within the Farmington city limits only. Minimum 24 hr. advance notice required. Call 505-325-3409 to make reservations. Reservations may be made up to 30 days in advance. Reservations for Dial-A-Ride and Paratransit services can be made any time during system operational hours and on Sundays for the following Monday. |
