Early Intervention services are available to all San Juan County families with children birth to three who exhibit a delay, may be at risk for a delay, or who are involved with the New Mexico Children's Youth and Families Department. A full developmental evaluation is completed to determine eligibility per New Mexico Family Infant Toddler regulations. A 25% or more delay needs to be noted in gross or fine motor skills, communication, self-help, or social emotional skills to qualify as Developmental Delayed . Children who are considered Medically at Risk for a delay are those that may have a vision or hearing loss, or whom are born premature or with a medical condition that could result in a developmental delay. Children with an established condition also qualify for services, these conditions consist of cerebral palsy, or Down's Syndrome for example. They are specific conditions that a child is born with and will cause a developmental delay. If a child's family is involved with the Children's Youth and Families Department, those children may qualify under the Environmental Risk category. Roundtree provides case management, special instruction, Occupational therapy, Physical therapy, Speech therapy, Nutritional services, and outreach services to New Mexico School for the Blind and Visually Impaired as well as to the New Mexico School for the Deaf. Transition services are also provided as part of the child's Individual Family Service Plan to assist with a smooth transition from Early Intervention services to the family's choice for their preschool/academic setting. Since 1982, Project Shield has been committed to responding to the developmentally disabled adult employment and community inclusion needs of San Juan County residents. The service area includes the communities of Farmington, Aztec, Bloomfield, the San Juan portion of the Navajo Reservation and all other areas of the County. Project Shield works with clients, guardians, and the community to provide a range of individualized employment and community inclusion related services to persons with developmental disabilities. Each client has an Individualized Service Plan that helps staff address the unique needs of each client and provide the most effective support possible. Services are provided through the following programs: New Mexico Developmental Disabilities Waiver. Day Habilitation: Habilitation services help maximize independent living skills in the areas of self care, physical and emotional maturity, and social communication. Staff is trained to assist clients in areas of money management, community integration, individual relationships, educational pursuits, personal care, and other areas identified in the clients Individualized Service Plan. Community-Supported Employment: This employment model provides job development, placement, on-the-job training, and on-going support to individuals in community employment. All clients receive service coordination and transportation services, as well as assistance from a Job Coach who consistently evaluates job placement and support satisfaction. Project Shield partners with over 20 employers in San Juan County, allowing the program to offer clients job placements with a wide variety of skill level. Community Access: Project Shield has an intensive program to identify and implement natural supports for all clients. Natural supports consist of non-paid volunteers or clients who provide friendship and social interaction to other clients. Often volunteers take clients to a movie or other social activity and assist in linking clients to resources in the community that promote independence. Non-Medical Transportation: Transportation services are provided to all clients to and from the Project Shield facility, employment settings, social activities, shopping, etc. |

Totah Behavioral Health Authority was founded in 2001 to address the problems associated with chronic alcoholism and drug abuse in Farmington and San Juan County, New Mexico. TBHA is collaboration between the three major governmental entities in the Four Corners region including the Navajo Nation, San Juan County and the City of Farmington, as well as six other partnering agencies who are the predominant providers of primary medical and behavioral health service in the area. These are San Juan Regional Medical Center, Farmington Intertribal Indian Organization, Four Winds Recovery Center, Presbyterian Medical Services, the State of New Mexico Department of Health and the Navajo Area Indian Health Service. TBHA offers the New Mexico Comprehensive Community Support Services model for case management that incorporates the Community Reinforcement Approach (CRA), individual intervention that is grounded on motivational interviewing (MI), an intensive outpatient services that uses the Matrix Model, and an extensive referral network that includes providers in California and Arizona. A vital part of our services include Traditional Native American healing practices based on the Navajo teachings and culture. BHA services are sensitive to the cultural and ethnic needs of our relatives, who are referred to Totah from local courts, the DWI residential program, street outreach, and health care agencies. Using traditional counseling, facilitated community support work and contemporary best practice behavioral therapies, TBHA brings relatives into a caring and supportive environment. Caseload: TBHA serves a total of between 125 and 175 relatives (clients) monthly with each case worker assigned to 20 to 26 cases. Approximately 75 % of TBHA relatives are male which is lower than in previous years as more women have come into the Totah program. The age of clients served ranges from 18 years to 69 years old, with the majority of those served aged between 20 and 40. At least 90 % of clients served at Totah Behavioral Health Authority identity their primary substance of abuse as alcohol. However, the numbers of people presenting with methamphetamine addiction are increasing at a steady rate. Primary DSM diagnoses presenting remain 303.90; Alcohol Dependence and 304.80; Polysubstance Dependence. Staff: TBHA has 14 staff positions including one substance abuse counselor, a traditional counselor, a clinical services director, six community support workers, two Peer Support Specialists, two clerical support staff, plus the Executive Director, Ray Daw. Totah's regular hours of operation are Monday through Friday, from 8:00 am to 5:00 am. The traditional program does periodically provide some activities and services on weeknights or weekends. Supportive Living:
Presbyterian Medical Services provides three core services to the seniors in the Thoreau community. The services provided by the PMS Thoreau Senior Center are vital to the senior population in these communities. • Congregate meals assure ambulatory seniors receive a nutritious daily meal including socialization. • Delivery of meals to homebound seniors assures nutritious meals; providing staff with the opportunity to conduct needs assessments, determining risk factors. Often this is only meal the senior member consumes for the day. • Transportation services are vital to the seniors who no longer drive and want to visit the center for meals, as well as seniors who may not feel comfortable driving the 70+ miles to 'town' for medical appointments and shopping, increasing the seniors' ability to live alone at home. |

PMS/Checkerboard Area Health System (CAHS) is a system of 5 clinics located in and around Cuba, N.M. We provide a large range of services, including, but not limited to diabetes education and management, hypertension management, chronic disease management, acute care issues, prenatal, pediatric, and immunizations. We have special contractual agreements which allow us to provide optometry care and high risk OB care. CAHS has a capitated contract with Indian Health Services to provide healthcare to all Native Americans who reside within our contract care service area. Through this contract we provide Public Health Nursing and Utilization Review for our IHS patients. We receive funding through a multitude of sources, including most private insurances, Medicaid, Medicare, Federal 330 grants funding, RPHCA funding, Indian Health Care funding, as well as, Sandoval County and State funding for the ambulance program. Dental: Our dental clinics provide care for both adults and children. Our dental services consist of oral health screenings, cleanings, basic and major restorative care, and denture services. |
